Summer Enrichment Programme students are required to fill this out by the end of the first week of the programme.

To be completed at the end of each Habit of Mind instruction period.

This survey is for Primary students of the Bermuda Public School System only.  Based on their Renzulli Profile, each child can select three (3) enrichment seminars to participate in after school over the course of the school year.  Enrichment seminars occur one afternoon per week, 3:30pm-5pm, for approximately nine (9) weeks each term.  There are three (3) terms in a school year (Fall=Sept.-Dec., Winter = Jan.-Mar., and Spring=Apr.-Jun.)
