Asking all parents/guardians of students with Asthma (ages 4-18) to please complete the linked survey below.

This survey/registry is a collaborative effort of the Department of Education, the Department of Health, the BHB Asthma Education Centre and Open Airways – all working together in various capacities to improve and assist these students to have the healthiest school experience possible. Completing the survey ensures the students name will be included on the Bermuda Schools Asthma Registry.
Our primary aim is to ensure every child with asthma receives the guidance and assistance they need with their asthma throughout the school year!
Statistics show that approximately 1 in 5 school children in Bermuda have asthma – 20%!
With proper treatment and control, these students should not be missing school due to their asthma AND should be able to participate in ALL school activities, including sports!
